Consent Forms
All facilities we serve keep a paper copy of the consent form on file.
You may request a copy by speaking with the nurse on the unit or the office administrator.
If preferred, a digital copy of the consent form can also be provided upon request.
What Do Foot Care Visits Include?
Advanced Nursing Foot Care provides non-invasive treatment and guidance for common foot concerns, including:
- Thick or possible fungal nails
- Non-infected ingrown toenails
- Heel fissures
- Calluses and corns
- Diabetic foot care
Foot care nurses typically visit homes on a six-week schedule to support ongoing care and monitoring.
Independent visits are not offered at this time. If an independent visit is required, a referral to a private foot care practitioner can be provided.
All care is delivered within the nurse’s professional scope of practice. If a condition requires specialized treatment, an appropriate referral will be made.

Facility Care Billing
Please refer to the consent form for current foot care service rates.
We prefer to bill the resident’s comfort, trust, or monthly account.
Receipts are available at the administration office after each visit.
Payment and Direct Billing
Payments are accepted by cheque or e-transfer
We do not provide direct billing to insurance companies
Please contact your insurance provider to confirm coverage
If you do not have insurance, keep your receipt — it may be claimed as a medical expense on your income tax
Additional Fees
Finger Nail Care: $15 per visit (add-on service)
Invoicing Fee: $2 per mailed document
Late Payment Fee: $5 if payment is not received within 30 days
Plus $2 for each additional mailed document
Please Note:
Foot care services may be placed on hold until any outstanding balance has been settled.

Coverage confirmation is the responsibility of the insurance holder.
